The health and safety of our campers is the primary concern of everyone at CYJ Sprout Lake. Therefore we must have the fully completed and signed Health Forms and Camper Profile for each camper prior to the start of camp (both a parent’s signature and physician’s signature is required). Our resident nurses and physicians carefully review these forms. Any special needs and considerations are communicated only with those staff members and medical professionals who we believe need to know about any condition that your child may have, or as required by law.
We understand parental fears about a child being “labeled” if they provide too much information, and we understand the desire to give your child an environment where he or she gets a fresh start with no prejudgment. Nonetheless, we ask that you provide us with all requested information about your child and his or her conditions. We want to treat your child properly and fairly and the only way to do that is with full knowledge of your child’s medical and/or mental health condition.
The Health History and Examination Forms (a multi-part form that is available in your CampInTouch account) must be completed by you and your health care provider and received by camp no later than May 15, 2022. No camper will be permitted into camp without the signed and completed Medical Form and a copy of your child’s health insurance and prescription cards on file in our Health Center.
Please answer all the questions carefully, especially those dealing with immunization data. Please make a copy of the health forms for your own files.
The Dutchess County and New York State Board of Health require that no camper be admitted to camp until properly signed medical forms and emergency treatment authorization have been received by the camp. There can be absolutely no exceptions to this rule! Make sure that both a parent and a physician sign the Health Forms. Incomplete or incorrectly completed forms will be returned and may cost your child his or her spot in our program. Please make sure these forms are completed correctly!
Glasses & Contact Lenses: If your child wears prescription eyewear, it is important that he/she brings an extra pair to camp. It is also recommended that a copy of the prescription be attached to your child’s health form.
Dental Needs: Be sure to notify us if your child should be wearing a retainer or night brace. Please send an adequate supply of all associated equipment and supplies (such as elastic bands and wax) with your child to camp. Camp is not responsible for any damages to retainers and night braces.
Medic-Alert Bracelet: Children who react to specific antibiotics or medications, or who have specific medical ailments, should wear a Medic-Alert bracelet, which can be obtained through the Medic-Alert Foundation in your city.
